Please download, fill out and bring with you on your first visit the New Client Registration Form and the attached Springhurst Animal Hospital Policies form. Both are required for all new clients.

Prior to a procedure requiring the use of anesthesia, we perform a full physical examination to identify pre-existing medical conditions that may cause complications. We require your consent to authorize further diagnostic testing, post-op pain medication, microchipping, etc.

This form includes generalized care instructions for your pet.

Please allow 24 hours for prescriptions to be refilled. For prescriptions needed immediately, please call (502) 326-2975. If a prescription cannot be refilled, you will be contacted via phone. Form processed during normal business hours.

Our boarding facility is a full-service operation. We now offer a fenced-in ½ acre play area for play times, day care and boarding. We offer pick-up and drop-off Mondays through Saturdays during business hours. Boarding fees include Eukanuba food, bedding, and multiple walks during the day on our business property.

Camp Springhurst also offer extras for your pet's comfort, such as additional playtimes, television time with a staff member, and snack times, at an additional cost.
